This plan gives you the freedom to receive treatment from any licensed dentist. However, you’ll save time and money when you use providers in the network, which includes thousands of licensed dentists and specialists nationwide.

Staying in-network also means you can save on the amount you pay in deductibles and other out-of-pocket expenses. And the in-network dentists offer more convenience—they typically handle the paperwork for you.

  • Preventive dental care for services like exams, cleanings and x-rays covered at 100%, with no deductible required.
  • Once you pay a deductible, the plan covers 90% of costs for basic services like filings, root canals, and gum treatment.
  • Our plan covers up to the $3,000 annual maximum for covered services.
  • Costs for major services, including crowns, inlays, and implants, are covered at 60%.
  • Orthodontic benefits for both adults and children are covered at 50%, up to a $2,000 lifetime maximum benefit.

Vision Plan

You and and your eligible dependents have access to comprehensive vision coverage. This plan provides great benefits on eye examinations and eyewear, plus other valuable discounts—all at an affordable price.

What you need to know

  • You and your dependents will receive quality care from an eye doctor. They’ll look for vision problems and signs of other health conditions in addition to assessing your vision for corrective lenses or treatment.
  • You can visit either in-network or out-of-network eye doctors, but you’ll save money and have lower out-of-pocket costs if you use a provider who is a member of the network.
