HDHP/HSA plan

Plan highlights

While you’ll pay a lower per paycheck premium for coverage, the HDHP deductible is relatively high, which means you’ll also be responsible for a larger share of the cost for your everyday medical expenses throughout the year.
However, the HDHP helps you keep your out-of-pocket costs down by pairing it with a special Health Savings Account (HSA), which allows you to pay for many of your expenses with pre-tax dollars, offering you a big tax break before you even spend the money on care.
You’ll also pay less by using in-network doctors, hospitals, and other providers. You are able to use providers and receive care outside of the network, but if you do, you’ll generally pay a bigger share of the cost.
After you meet your deductible, the plan generally pays 90% for in-network office visits and eligible healthcare costs.
In-Network preventive services are 100% covered.

PPO plan

This plan has copays for office visits and prescription drugs, and the copays apply even before your deductible is met!
After you meet your deductible, the PPO plan generally pays 90% of eligible in-network health care costs for things like imaging, hospital admissions, and surgeries.
This plan includes out-of-network coverage. But, if you see an out-of-network provider, you are generally responsible for a greater share of the cost.

EPO plan

Plan highlights

This plan has no deductible and features flat, predictable copay amounts for most services.
In-network preventive services are 100% covered.
This plan has extremely limited out of network coverage. It is best to stay in-network at all times if you’re considering the EPO.
